The Regulated Member and the College entered into a Disciplinary Complaint Resolution Agreement on March 6, 2025.

The Regulated Member agreed that in or around November 2023 – January 2024, he engaged in unprofessional conduct in that, between November 2023 – January 2024, the Regulated Member failed to adequately document their SLP care for four (4) acutely ill patients, including regarding observations and rationale for diet changes and dysphagia management.

The conduct admitted to in the DCRA arises from one complaint. The Regulated Member agreed to provide a self-reflective paper on the importance of documentation that includes areas and goals of improvement. The Regulated Member must also complete a period of six (6) months of monitoring in the workplace, including undergoing documentation audits by his employer and confirmation that his documentation meets the minimum expectations of ACSLPA.
